Every woman should plan a visit to the doctor once a year for a physical exam. You want to see a doctor who considers you a whole person, not just a “gynecological exam.” All of your systems are involved in your total health and all must be considered.
The first part of your annual exam should include your “History”. You will probably fill out a detailed sheet looking for clues about your health. Have you lost or gained weight? Do you sleep well Have you been feeling depressed? Do you have headaches? Has your vision changed? Do you have chronic sinus problems or a sore throat? Have you noticed lumps on your neck or other areas? Are you short of breath? Does your chest hurt? Do you do breast self-exams (hopefully) and, if so, have you found lumps or other irregularities? When was your last mammogram? Have you had abdominal pain or intestinal irregularities? When was your last period? It was normal? Have you ever had pelvic inflammatory disease (PID)? Do you have hot flashes? How are your joints? Do you have calf pain? Have you ever had a blood clot? Have you had any surgery since your last exam? All of these questions and more are very important.
The second part of the exam consists of your vital signs: Have you lost height, perhaps due to osteoporosis? Has your weight changed? Most of the time we have a tendency to gain weight as we age, but problems such as thyroid conditions or diabetes could also be a cause of weight change. You may need a diet to lose weight and a prescription for exercise. What is your pulse? Is it too fast, as with an overactive thyroid or a heart problem? Is your heart irregular? You may need an EKG. Finally, what is your blood pressure? We all know that the long-term effects of high blood pressure are serious and you may need medicine for this.
The next part of your exam is the actual physical. Do your eyes, ears, nose, and throat appear normal? Do you have lumps in your neck, abnormal sounds in your carotid arteries, or an enlarged thyroid? How do your lungs sound? Is there wheezing or other abnormal sounds? How does your heart sound? Are there heart murmurs or is there any evidence that your heart is enlarged? How does your abdomen feel? Are there sensitive areas, particularly over the gallbladder or spleen area? Looking at your back, does the spine appear straight or is there a developing curvature as in osteoporosis? Are you sensitive about the kidneys? How do your legs look? Are there varicose veins or swelling of the ankles? Are the pulses full and strong at the junction of the foot and ankle, and are the reflexes in the legs normal?
The next part of the exam is the breast exam. The doctor will examine you both sitting and reclining. It will look for unusual dimples, lumps, unusually tender nipples, or nipple discharge. He will carefully feel under your arms for swollen or hard lymph nodes. At this time, they will let you know if you are in the age group to have a mammogram, or if your breast findings are such that a mammogram would be advisable. There is a certain amount of radiation involved in a mammogram, so this decision should be made wisely. Your doctor will decide if you need to be referred to a surgeon for a biopsy or lumpectomy.
The next part of the exam is the gynecology exam. The doctor will examine your external genitalia to look for growths, cysts, or areas suspicious for cancer. The clean speculum instrument will then be inserted with a clear lubricant that allows the vagina and cervix to be examined. Again, the doctor looks for any evidence of abnormal tissue or infection. He will gently take a scraping from the vaginal canal of the cervix for the Pap test and from the opening of the cervix. These scrapings will be placed on a glass slide and will be read by a pathologist under a microscope. At certain ages and risk groups, you will take cultures from the same area to check for chlamydia or gonorrhea. This is very important to prevent PID. The doctor will do a “bimanual exam,” with one hand on your abdomen and two fingers of the other hand on the vagina. This helps detect if you have any pain, mass, or any anatomical change. This allows the doctor to determine if your uterus or ovaries are enlarged or hardened, or if you have any pain, and if more tests are needed, such as an ultrasound. If the cervix is sensitive to back and forth movement, this could suggest PID.
The final part of this exam is usually a digital rectal exam. The doctor will look for hemorrhoids, abnormal growths (ie, possible signs of anal cancer, etc.), and perform a slide test for rectal occult blood.
Finally, the doctor will recommend laboratory tests. Many doctors recommend annual HIV tests, an RPR for syphilis, and tests for chlamydia and gonorrhea; but you can decide this together with your doctor. He or she may recommend a blood count to check for anemia or other blood problems, and often a complete chemical test will check everything from liver function to cholesterol. If thyroid problems are suspected, a T3, T4, and FTI may be recommended. Again, this is all for discussion between you and your doctor based on your age group, general health, history, and physical exam.
